Good Faith Estimate

Your right under the No Surprises Act to a written cost estimate before scheduled care — what's covered, how to request one, and what to do if your bill exceeds it.

Overview

At TruForm Longevity Center, we believe in transparency and want you to understand your rights regarding the cost of your medical care. Under the No Surprises Act, healthcare providers must provide patients who don't have insurance — or who are not using insurance — with estimated expected charges for medical items and services.

Your Rights

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency integrated functional and regenerative medicine care, including related costs such as medical tests, equipment, and other associated fees.
  • You may request a Good Faith Estimate before you schedule a service by contacting our patient services team at 207-780-2000.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
  • Please save a copy or take a picture of your Good Faith Estimate for your records.
